2- Physical Products:

In consideration of being allowed to participate in Seazon Youth Sports Association athletic programming and related events and activities (collectively referred to as “SZN Sports”), the agreeing party acknowledges, appreciates, and agrees that:

Participation in SZN Sports consists of involvement in group activities, which involvement includes possible exposure to, and illness resulting from, communicable diseases, infectious diseases and/or transmittable diseases including, but not limited to, influenza and COVID-19;

While following social distancing and similar rules, as well as exercising personal discipline, may reduce this risk, the risk of serious illness, injury, disability, and death from such diseases (including, but not limited to, influenza and COVID-19) does exist;
By participating in SZN Sports, I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the Releasees (defined below) or others, and assume full responsibility for my participation or the participation of my minor child/ward, including as it relates to any diseases, including but not limited to, influenza and COVID-19;

I willingly agree to comply with the stated and customary rules, terms and conditions for participation, including in regards to protection against communicable, infectious and/or transmittable diseases. If, however, I observe any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of SZN Sports immediately; and,

I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release and hold harmless SZN Sports, it officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct any and all activities of SZN Sports (“Releasees”), with respect to any and all illness, injury, disability, death, or loss or damage to person or property, whether arising from the negligence of Releasees or otherwise, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND AGREE TO IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

Photo Release *

For good and valuable consideration, I hereby grant SZN Sports permission to use my likeness in a photograph in any and all of its publications, including but not limited to all of SZN Sports printed and digital publications. I understand and agree that any photograph using my likeness will become property of SZN Sports and will not be returned. Images will not be resold or reused for any use other than SZN Sports. I acknowledge that since my participation with SZN Sports is voluntary, I will receive no financial compensation. I hereby irrevocably authorize SZN Sports to edit, copy, exhibit, publish or distribute this photo for purposes of publicizing SZN Sports programs or for any other related, lawful purpose. In addition, I waive the right to inspect or approve the finished product including written or electronic copy, wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photograph. I hereby hold harmless and release and forever discharge SZN Sports from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
